975,065 notices were sent wrongly with another person's basic pension number, bank, payment amount and deductions printed beside the addressee. By prefecture: Aichi 804,033, Fukuoka 166,026, Wakayama 3,041, Mie 1,964 and Nara 1. The other person's name was not printed. [1] [3]

Three causes were found at its contractor Sunmesse. (1) A test product made in a print environment different from production was submitted as if made in the same one. (2) The operator set the printer output wrongly and no one checked it. (3) The control numbers on the front and back were not cross-checked. [5]
